鼻腔跟鼻竇是防禦病毒、細菌入侵呼吸道的最前線,當這最前線的免疫反應崩盤時,很可能會導致惱人的慢性鼻竇炎。慢性鼻竇炎不是個會讓人感到陌生的醫療名詞,而這也的確反應在罹病人口上。在美國有超過百分之十的人為此所擾,一年相關的醫療花費高達八十億美元。而台灣因為氣候濕熱,慢性鼻竇炎患者更估計高達一成五到兩成。如果是細菌感染而導致的鼻竇炎,目前多半靠給予抗生素來治療。美國境內的統計數據顯示,約每五張抗生素處方箋裡,就有一張是拿來治療鼻竇炎。這麼多數據所勾勒出來的慢性鼻竇炎樣貌實在嚇人。尤其,現在什麼抗生素都不怕的超級細菌讓人驚恐,每一張抗生素的處方箋都應該用在刀口上,更讓醫療研究界想要好好研擬除了抗生素之外,能夠拿來對付慢性鼻竇炎的絕招。

而最近,科學家將腦筋動到了苦味、跟甜味的味覺感受器上。

上呼吸道感染,和苦味與甜味的味覺感受器之間的關聯,乍聽下八竿子也打不著,但實際上卻是唇齒相依的關係。舌頭上的味覺細胞表現味覺感受受器是天經地義,但其實味蕾卻不是唯一有味覺感受受器這個秘密武器的細胞。像是腸道、胰臟、大腦,甚至骨頭與脂肪組織,都可以找到甜味味覺感受器的蹤跡。而在上呼吸道上,有一種「單一型化學感受細胞」(solitary chemosensory cells,常被叫做 SCC 細胞),同時具有甜味與苦味這兩種味覺感受受器。

呼吸道專職呼吸,SCC 細胞為什麼要特地表現甜味跟苦味味覺感受器呢?科學家發現,甜味跟苦味這兩種味覺感受器,在 SCC 細胞上不單單只是放著好看的花瓶,而是 SCC 細胞用來抵抗細菌感染的獨門絕技。

最先引起科學家興趣的是苦味味覺感受器。如果苦味味覺感受器被刺激、活化,會分泌抗微生物胜肽(antimicrobial peptides)。胜肽一般泛指小片段的胺基酸序列,而抗微生物胜肽「職」如其名,在人體通常是 30 個左右的胺基酸所組成,可以藉由破壞微生物細胞膜表面通透性,而達到毀「膜」滅菌的目的,是一種從植物、果蠅、魚類等上至會飛的,下至會游的生物都擁有的抗菌法寶。

而上呼吸道 SCC 細胞所表現的苦味味覺感受器,在偵測到植物衍生的有毒物質(例如生物鹼),或是細菌之間分泌用來彼此溝通的化學物質時,會被活化。一旦苦味味覺感受器被啟動,就會使 SCC 細胞與周圍的表皮細胞分泌抗微生物胜肽。 162단어 남음
